As an RN Hospice Case Manager on the East side, you will be an invaluable resource to our organization, providing expert, end-of-life care to patients with terminal illnesses.

Responsibilities

Clinical Essential Functions:

  • Patient Care: Provide prescribed medical treatment and personal care services to patients in both home and inpatient settings.

  • Nursing Process: Utilize the principles of the nursing process to assist with data collection, care planning, and evaluation of nursing interventions.

  • Therapeutic Nursing Care: Demonstrate clinical knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care.

  • Vital Signs: Take and accurately record patients' vital signs.

  • Medication Administration: Administer prescribed medication (orally, subcutaneously, or intramuscularly), ensuring proper documentation of time and dosage in the patient’s chart.

  • Patient Education: Reinforce patient care instructions and explanations, including the Plan of Care or any relevant teaching plan.

  • Communication: Effectively communicate both orally and in writing to convey pertinent information to the healthcare team in a timely manner.

  • Health Status Reporting: Report any significant changes in the patient’s condition or health status to the appropriate team members promptly.

  • Patient Safety: Maintain a safe environment for both patients and self, adhering to safety protocols.

  • Learning Needs: Identify unmet learning needs in patients or families and seek assistance from other healthcare professionals to address them.

  • Compliance: Follow all company policies and procedures in daily tasks.

  • Documentation: Complete all required forms, reports, time cards, itineraries, etc., accurately and within designated timeframes.

  • Communication Protocol: Recognize and adhere to established lines of communication for reporting and collaboration.

Qualifications

Qualifications and Requirements:

  • Education:

    • Completion of an accredited RN education program with a diploma.
    • Associate’s or Bachelor’s degree required; Bachelor’s degree in Nursing is preferred.
  • Experience:

    • Minimum of one year of hospital experience.
    • At least one year of hospice, home care, and/or community health experience is required.
    • Experience working effectively as part of a team, with individuals and communities from diverse cultural, social, and ethnic backgrounds.
    • Ability to thrive in a complex institutional structure that requires significant collaboration.
  • Skills:

    • Ability to work independently with minimal supervision.
    • Strong ability to maintain strict confidentiality.

Required Licenses, Registrations, and Certifications:

  • Nursing License: Active RN license in the state of Ohio.
  • CPR Certification: Must be current and valid.
  • Driver’s License: Valid Ohio driver’s license required.
