PRN Physical Therapist Home Health Weekends

 

At Visiting Nurse Association of Ohio, we are looking for dependable, compassionate people interested in gaining experience and becoming an invaluable resource to our organization. VNA of Ohio is hiring a Physical Therapist and your qualifications and skill set may be an ideal match for this opportunity.  This position is PRN supporting the Eastern Suburbs.

Responsibilities

The Physical Therapists will be responsible for providing home care therapy to individuals that help restore function, improve mobility, and relieve pain.  Including but not limited to the following tasks:

  • Performs initial assessment or admission to agency to determine patient treatment needs.
  • Develops a plan of care with measurable goals and specific discipline related treatments in conjunction with physical referral/orders.
  • Provides direct physical therapy services as outlined in the plan of care including performance of physical therapy procedures, instruction of patient and/or caregiver, recommendations for equipment or adaptation of home environment.
  • Re-evaluates and revises plan of care as needed with physician approval.
  • Facilitates total care services to patients by making appropriate referrals to other disciplines by obtaining physicians’’ orders or by consulting with case manager.
  • Submits all documents in a timely manner.
  • Obtains physician approval to modify a current plan of care or treatment program.
  • Performs other related duties as assigned.

Qualifications

Qualified candidates must:

  • Bachelor’s Degree in Physical Therapy preferred.
  • One year experience in a supervised healthcare setting.
  • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.  The ability to work in a complex institutional structure requiring significant collaboration. 
  • Strong planning and organizational skills.  
  • Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to maintain strict confidentiality. 

 Education/Training/Experience:

  • Licensed Physical Therapist by the State of Ohio.
  • CPR certification required.
  • Valid Driver’s License
