Full-Time LPN for West side Cuyahoga Territory

The Visiting Nurse Association of Ohio is seeking an experienced and motivated Licensed Practical Nurse (LPN) to join our dynamic team serving the West side Cuyahoga area. This is a full-time position offering the opportunity to work in a supportive, community-focused environment that values your expertise.

 

As an LPN with VNA of Ohio, you will play an essential role in providing high-quality patient care in the comfort of their homes, ensuring that each patient receives the personalized treatment and attention they deserve. 

Responsibilities

Key Responsibilities:
  • Deliver Direct Patient Care: Provide prescribed medical treatment and personal care services in home or inpatient settings under the guidance of a Care Manager or Central Monitoring Nurse.
  • Nursing Process: Assist in data collection, care planning, and evaluation of nursing interventions.
  • Patient Support: Administer medications (oral, subcutaneous, or intramuscular), record vital signs, and monitor patient progress.
  • Communication: Maintain clear, timely communication with the healthcare team and families regarding care plans, changes in patient health status, and important patient information.
  • Safety & Compliance: Ensure a safe environment for patients and yourself, comply with all policies, and complete necessary documentation (reports, timecards, itineraries) in a timely and accurate manner.

What We’re Looking For:

  • Experience: Preferred background in home care or community health; experience with infusion therapy is a plus (required for inpatient care).
  • Skills: Ability to work independently, follow established communication lines, and collaborate effectively within a team.
  • Cultural Competence: Experience working with diverse populations, embracing different cultural, social, and ethnic backgrounds.
  • Confidentiality: Demonstrated commitment to maintaining patient confidentiality.

Qualifications

Qualifications:

  • Licensure: LPN certification licensed in the State of Ohio.
  • Education: High school diploma or GED; graduate of an accredited LPN program.
  • Experience: At least 1 year of LPN experience, with a minimum of 6 months in a hospital or LTAC setting.
  • Certifications: CPR certification required.
  • Driver’s License: Valid Ohio driver’s license required.
