Full-Time Physical Therapist – Home Health

 

At Visiting Nurse Association of Ohio (VNA), we are seeking dedicated, compassionate professionals interested in gaining valuable experience while making a meaningful impact. VNA of Ohio is currently hiring a Full-Time Physical Therapist in the West side of Cuyahoga County to join our team.

 

As a Physical Therapist, you'll assess and evaluate patients' mobility, endurance, pain, and other medical conditions. You’ll create personalized treatment plans, provide therapy, and track patient progress to improve their well-being.

Responsibilities

Key Responsibilities:

  • Patient Assessment: Conduct thorough assessments to understand each patient’s mobility, endurance, pain levels, and other needs.
  • Treatment Planning: Develop personalized treatment plans in collaboration with physicians and healthcare teams.
  • Therapy Services: Deliver hands-on physical therapy, teach patients and caregivers exercises, and recommend home modifications or equipment.
  • Progress Monitoring: Regularly evaluate patient progress and adjust care plans as needed.
  • Team Collaboration: Work closely with other healthcare professionals to ensure comprehensive care.
  • Documentation: Ensure all patient records and treatment plans are up-to-date and submitted on time.

Qualifications

Qualifications:

  • Degree: Bachelor’s degree in Physical Therapy required (Post-graduate degree preferred).
  • Experience: Minimum of 1 year of clinical experience in a healthcare setting.
  • Licensing: Licensed to practice physical therapy in the State of Ohio.
  • Certifications: CPR certification required.
  • Other: Valid driver’s license.
