In this dynamic role, you’ll bridge the gap between healthcare professionals, patients, and our services, all while helping our hospice care programs thrive.

Responsibilities

What You’ll Do:

  • Coordinate Care: Receive home care orders, complete patient assessments, and gather everything needed to ensure smooth, efficient referrals.

  • Spread the Word: Share the benefits of VNA’s comprehensive services with healthcare partners, building relationships and driving referrals.
  • Engage the Community: Identify and create opportunities for educational programs, health fairs, and CEU presentations to raise awareness about hospice and home care in your community.

Requirements:

  • Active Nursing license in the State of Ohio.
  • CPR certification.
  • Valid driver’s license.
