At the Visiting Nurse Association of Ohio, we’re seeking compassionate and dependable individual to join us a RN Case Manager Home Health in Medina and surrounding areas. The RN Case Manager will be responsible for providing direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups.  This full-time role is an excellent opportunity for those looking to gain valuable experience and make a significant impact in healthcare.

Responsibilities

Key Responsibilities:

  • Utilize the nursing process for data collection, planning, and evaluation of care.
  • Implement therapeutic and rehabilitative nursing care with a focus on patient safety and comfort.
  • Effectively communicate and collaborate with healthcare providers, patients, and families.
  • Observe and document patient conditions and reactions, promptly notifying the appropriate healthcare team members.
  • Respond to life-saving situations according to established protocols.
  • Ensure timely communication of relevant information to the healthcare team.

Qualifications

Qualifications:

  • Experience: Home health care experience preferred; 1-2 years in Med-Surg or hospital setting.
  • Education: Accredited RN education program completion; Bachelor's degree in Nursing preferred.
  • Licensure: Registered Nurse licensed in Ohio; valid driver's license and CPR certification required.
  • Skills: Strong planning, organizational, and communication skills; ability to work under pressure and maintain confidentiality.
  • Additional: Experience working with diverse communities; bilingual skills a plus.
