The Visiting Nurse Association of Ohio is seeking a dedicated RN Case Manager to join our team in the East Cuyahoga territory. This full-time position involves providing comprehensive nursing care to patients across various age groups and health conditions.

Key Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Provide preventive, therapeutic, and rehabilitative nursing services effectively.
• Engage with patients and families through effective communication and interviewing techniques.
• Collaborate with healthcare providers to ensure coordinated care.
• Monitor patient comfort and safety, documenting significant changes and reporting them as needed.
• Respond promptly to emergencies following established protocols.
• Share relevant information with the healthcare team in a timely manner.

Qualifications:

• Experience: Minimum of 1-2 years in Med Surg or a hospital setting; home health care experience preferred.
• Education: Completion of an accredited nursing program (diploma, Associate's, or Bachelor's degree; Bachelor's preferred).
• Licensure: Registered Nurse licensed in the State of Ohio.
• Skills: Strong organizational and planning abilities, attention to detail, and the ability to work under pressure.
• Independence: Capable of working autonomously with minimal supervision while maintaining confidentiality.
• Other: Valid driver's license and CPR certification required; bilingual skills are a plus.
