The VNA of Ohio is currently seeking a PRN Speech Language Pathologist for Western/Central Cuyahoga territory!

The Speech Language Patholigist will be responsible to provide service to homebound patients with speech and language problems.

Responsibilities

Essential Functions:

Illustrative duties may include, but are not restricted to, one or more of the following:

  • Performs initial assessment or admission to agency to determine treatment needs of patient within home environment on physical referral only.
  • Develops a plan of care with measurable goals and specific discipline related treatments in conjunction with physician referral/orders.
  • Provides direct speech therapy services as outlined in the plan of care including performance of speech therapy procedures, instruction of patient and or caregiver, recommendations for equipment or adaptation of home environment.
  • Re-evaluates and revises plan of care as needed with physician approval.
  • Performs scheduling and record keeping procedures in accordance with agency policy.
  • Performs other related duties as assigned.

Qualifications

Qualifications and Requirements:

  • Masters Degree in Speech Language Pathology.
  • Two year minimum of recent experience in Speech Therapy including direct service to patients.

·       Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.  The ability to work in a complex institutional structure requiring significant collaboration. 

·       Strong planning and organizational skills.  

·       Ability to work independently with minimal supervision.

·       Demonstrated ability to maintain strict confidentiality. 

 

 

Required Licenses, Registrations and/or Certifications:

  • Licensed by the State of Ohio.
  • Certificate of Clinical Competence from the American Speech and Hearing Association. 
  • CPR certification required. 
  • Valid Drivers License
