VNA of Ohio is now hiring a Home Health Aide for our Hospice Group and your qualifications and skill set may be an ideal match for this opportunity. This position is full-time.

    Responsibilities

    The Home Care Aide is an unlicensed paraprofessional healthcare worker, specifically oriented to assist in the care of the terminally ill under the guidance and supervision of a Registered Nurse. The Home Care Aide is directly responsible to the Home Care Aide Supervisor for day-to-day scheduling, to the Case Manager for specific plans of patient care and direct field supervision and to Clinical Manager for evaluation.

     

    Essential Functions:

    Illustrative duties may include, but are not restricted to, one or more of the following:

    • Assists with personal hygiene and activities of daily living in the manner described by the Home Care Aide Plan of Care.
    • Plans, prepares and/or assists with nutritious meals in accordance with prescribed diet.
    • Performs or assists with activities necessary to maintain a healthful, safe environment for the patient.
    • Performs prescribed treatment within limits of education and experience, under the supervision of a Registered Nurse.
    • Observes and reports changes in the mental or physical status of patient, reports these changes promptly to the Registered Nurse and documents them appropriately.
    • Requests appropriate assistance if unable to competently perform assigned tasks.
    • Records on appropriate forms activities performed, observations made and care provided at the patients home in a timely fashion.
    • Willingly accepts new patient assignments and efficiently substitutes for other Home Care Aides in the event of absence.
    • Willingly participates in patient care conferences, in-service programs and support groups.
    • Promotes customer service.
    • Accurate logs and reports on activities performed, observations made and care provided at the patients home.
    • Promotes mental alertness and interpersonal relationships with client by providing stimulation through social interactions.
    • Other duties as assigned by the Manager, Patient Care Services.

     

    Qualifications

    Qualifications and Requirements:

    • High School Diploma or GED.
    • Twelve months in a supervised healthcare setting.
    • Hospice experience preferred

     

    Required Licenses, Registrations and/or Certifications:

    • STNA Certification. (Existing VNAC non-certified staff will be grandfathered)
    • Must have 12 hours of training per year.
    • Valid Driver’s License
    • CPR certification
    • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration. 
    • Ability to work independently with minimal supervision.
    • Demonstrated ability to maintain strict confidentiality.
