    Responsibilities

    The Clinical Manager will be responsible for directing and managing the delivery of nursing and/or home care aide services.  Direct and manage the delivery of clinical services based on patient specific population or program inclusive of nursing, rehabilitation services, social work services, home care aide services or other disciplines supportive to clinical care. 

    Duties to include but not limited to the following tasks:

    • Directs, supervises and evaluates clinical and support staff including monitoring of productivity, case conferences, joint visits, multidisciplinary conferences and performance appraisals.
    • Maintains adequate daily work force through coordination of case assignment, utilization of supplemental staffing, approval of overtime, approval of time off requests and collaboration with other managers.
    • Coordinates staff needs for after hours and participates in after-hours clinical oversight in rotation with other managers.
    • Communicates with third party payors for appropriate adherence to contractual obligations within the scope of clinical delivery.
    • Responsible for meeting team clinical and financial outcomes and takes appropriate corrective action when required.
    • Promotes customer satisfaction through effective communication with patients, families and/or health professionals. • Convenes and facilitates regular interdisciplinary or team meetings, and case conferences.  Assist in the development of and adherence to a Plan of Care.
    • Maintains knowledge of Horizons electronic medical records system for care delivery applications and management reports.
    • Maintains and enhances professional development through appropriate education/organizational activities.
    • Maintains knowledge of requirements of regulatory, licensing, accreditation and/or partner agencies including, but not limited to, JCAHO, OSHA, ODH, and/or Cuyahoga County Community Health Board.  Participates in regulatory audits and assists in ensuring recommendations are implemented.
    • Collaborates with Quality and Staff Development staff to assess training, education and orientation needs of clinical staff and ensure compliance with goals, standards, etc.  Assists with orientation, competencies, and in-service programs for clinical staff.
    • Collaborates with Associate Chief/Chief of Clinical Practice and/or Human Resources regarding employee performance issues.
    • Participates in the identification of collaborative opportunities concerning program development, marketing of programs, and/or community activities to promote the VNA and its programs.
    • Performs other duties as assigned.

    Qualifications

    Qualified candidates must have:

    • Computer proficiency in Microsoft Office.
    • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.  The ability to work in a complex institutional structure requiring significant collaboration.
    • Strong planning and organizational skills.
    • Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
    • Ability to work independently with minimal supervision.
    • Demonstrated ability to maintain strict confidentiality.
    • Demonstrated effectiveness in written and oral communication.

    • Must be able to work Saturday and Sunday with oncall availability.

    Education/Training/Experience:

    • Bachelor’s Degree in related field.  Equivalent experience may be considered for selected positions.
    • Five years management, functional leadership and/or supervisory experience.
    • One year medical/surgical experience.
    • One year of experience in specialty practice for populations such as pediatrics, hospice, and mental health
    • Registered Nurse certification in the State of Ohio
    • CPR certification required
    • Valid Driver’s License.
