    If you are currently an Advanced Practice Nurse and you are looking for additional income, or wanting a more flexible schedule, the Visiting Nurse Association of Ohio is looking for Advance Practice Nurses for our Cleveland (Cuyahoga/Lorain/Lake Counties) and Mid-Ohio (Crawford/Huron Counties) locations.  The positions are PRN allowing you to work flexible hours at your discretion.  Interested candidates must be able to travel throughout specific territory as the position involves making post-partum home visits.  Ideal candidates should have experience in family practice with women's health experience preferred.

    Job Summary:

    Provides direct care and services for patients, including assessment, coordination of services, and referrals and medical management. Collaborates with physicians on treatment.

    Essential Functions:

    Illustrative duties may include, but are not restricted to, one or more of the following:

    • Provides health care maintenance through the delivery of episodic and routine visits and contacts; includes completing a history and physical assessment, diagnostics, screenings and environmental assessment.
    • Utilizes effective interviewing techniques; sets limits, and collects data based on specific role.
    • Maintains timely and accurate documentation of patient visits/contacts; completes all medical forms; verifies medication and order.
    • Incorporates use of clinically-based technology in patient care.
    • Adheres to agency policies, procedures and protocols; complies with state, federal and accrediting regulations and requirements.
    • Coordinates care with physician, home care or hospice staff, patients and families, specialists, pharmacies, and others as appropriate.
    • Assists in making referrals as appropriate.
    • Works closely with various agency personnel to assure comprehensive and effective case management.
    • Uses oral and written communication to convey pertinent information to members of the health care team in a timely manner.
    • Recognizes and follows established lines of communication.
    • Provides on-call services as necessary.
    • Demonstrates and maintains clinical expertise in providing medical care and oversight to the patient.
    • Perform other related duties based on departmental need.


    Qualifications and Requirements:

    • Master's degree in nursing.
    • Five years’ clinical experience and two years’ experience in nursing or as an APN.
    • Experience in home health, family practice or women's health care is preferred.
    • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration.
    • Strong planning and organizational skills.  
    • Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
    • Ability to work independently with minimal supervision.
    • Demonstrated ability to maintain strict confidentiality.  


    Required Licenses, Registrations and/or Certifications:

    • Registered Nurse; licensed in the State of Ohio.
    • Certification in an Advance Practice Nurse Specialty by a National Certifying Board.
    • Current Certificate of Authority to practice in the State of Ohio.
    • CPR certification is required.
    • Valid driver's license.
    • Prescriptive Authority preferred.
